2. RELEASE/WAIVER: I hereby agree to indemnify and hold harmless Noir Lux Candle Co, LLC and its employees against any and all claims for personal injuries or damages of any kind arising from participation in any Candle Pouring workshop, classes, events, and their resulting products.
3. ACKNOWLEDGEMENT: I fully understand and acknowledge that the workshops offered at Noir Lux Candle Bar may involve inherent risks, dangers and hazards which may result in injury or illness. I hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify Noir Lux Candle Bar, its managers, members, and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, or otherwise, which may arise from my participation in any workshops or classes, resulting products from workshops or classes, or from use of equipment provided by Noir Lux Candle Bar. I accept full liability for creations made by the participant in the workshop.
4. PHOTO CONSENT: By checking this box, you agree that Noir Lux Candle Bar may use the below named participants photograph in the routine promotion of its activities.
